Form Screen
The Form screen enables you to collect any other information or details from your learners. This is a particularly useful screen especially for public modules if you wish to obtain particulars from your anonymous learners. You can add up to 5 fields in the screen, customise each field label, and its respective placeholder text.
1. Optional. Toggle Yes/No to enable optional or mandatory responding from your learners.
2. Field label. Give your field a label (e.g. Name, Age, Job Role)
3. Field placeholder text. Give your field a placeholder text as an additional description or example of information required for the particular field.
4. + Add new form field. Add additional field(s) to your form screen (up to 5 fields).
